Were the findings demonstrated in systems in which growth was suppressed either by unfavorable temperature or by the use of depleted medium. However, in the former experiment in which it was shown that maximum killing occurred at the optimum growth temperature, another interpretation can be justly made. It is possible that if the antibacterial agent is an enzyme, the temperature of its maximum activity may be nearly identical with the optimum temperature for the growth of the culture. It was necessary, therefore, to test the effect of the saliva on organisms held in a resting state by some other method. Depleted media served this purpose well, wherein it was shown that the salivary factor exerted no apparent killing effect on the organisms. Moreover, when this was controlled by reconstituting the medium by the addition of glucose, the growth of the organisms and the action of the antibacterial agent were similar to the results observed in the complete medium. The results indicate that the salivary lactobacillus bactericidin exerts a lethal effect on sensitive cells only when they are growing. The action of this salivary component upon non-dividing organisms, on the other hand, was without obvious effect. In this respect, the mode of action of this antibacterial factor is similar to other antibacterial agents, such as penicillin, spermine, and polylysine.9-11 Although death due to lactobacillus bactericidin apparently results from unbalanced growth, no evidence is presented suggesting what the basic biochemical impairment may be. Salivary antilactobacillus systems with different physical properties have been reported in the literature. 13 The presence of more than one lactobacillus bactericidin in the saliva is a distinct possibility. Continued studies should be done to clarify this problem. It has been shown that a minimum amount of lactobacillus bactericidin is necessary for lethal action. Under natural conditions, saliva bathes the oral mucosa as a thin film, and it is possible that sufficient concentrations of the factor are not present to have a detrimental effect on susceptible micro-organisms. However, Williams and Powlen, 14 using a roller-tube technique to simulate the in vivo condition, reported that Lactobacillus, as well as other micro-organisms, was unable to survive and multiply in sterile parotid saliva. It has been established that the oral microbial flora is relatively characteristic for an individual.'5' 16 It is postulated that salivary antibacterial systems such as the one described in the present investigation may contribute to the regulation of the oral flora and play an important part in the natural defense mechanisms of the oral cavity. Since this agent probably represents only one of a large number of factors contributing to the antimicrobial defense system of the oral cavity, allied mechanisms must not be overlooked and caverject.

Adults, freshly drawn into heparin, was washed three times with 145 mM NaCl at 4C; the red cells were diluted to approximately 40% hematocrit and used the same day. Hemolysates were prepared by freezing and thawing the cell suspension. The fractional water volume, v, of the red cells in the reaction suspension was calculated as the hematocrit of the stock cell suspension water content of red cells [0.61 1 ; for the Philadelphia experiments and 0.65 4 ; for the Hannover experiments] its dilution in the reaction mixture. In later experiments, v was calculated as 55.5 [cyanmethemoglobin] in mM in the reactant mixture. NaHCO3 was prepared by incubating 2% and 5% 18O labeled HOH with unlabeled NaHCO3 at 150C in a pressure bomb for several days, after which the water was removed by lyophylization. DIDS was obtained from Research Organics and phlorizin phloridzin ; was obtained from Sigma. The technique of the 18O exchange method for measuring A and PHCO 3 has been described in detail 1, 5, 6 ; , so only a brief summary will be given here. The reactions take place in an airtight 3-ml glass water-jacketed and stirred chamber connected to the ion source of a mass spectrometer through a thin 0.012-mm-thick Teflon membrane supported by a sintered glass disc, through which gases are dissolved in the solution diffuse. The chamber has a glass pH electrode and a removable stopper for the addition of reactants. For experiments done in Philadelphia, the mass spectrometer was at first a Model 21 620A Consolidated Electrodynamics, Pasadena, CA ; and later a Model CH7 Varian MAT, which recorded masses 32 16O2 ; , 44 12C16O2 ; , and 46 12C18O16O ; peak every 22 sec on a strip chart. For experiments done in Hannover, the mass spectrometer was a Europa 1 stable isotope detector Europa Scientific, Crewe, U.K. ; , which had three separate detectors tuned to record masses 44 12C16O2 ; , 45 13C16O2 ; , and 46 12C18O16O ; simultaneously, and a computer program that calculated and recorded the ratio [12C18O16O] [12C16O2] once per second. The response time of the whole system to a change in 12C18O16O2 in the reaction mixture was about 3 sec, including a time delay. To initiate an experiment, sufficient dry 18O-labeled NaHCO3 to produce a 25-mM solution was added to 120 mM NaCl in the chamber. pH was adjusted within several minutes, and an aliquot of red cell suspension or of hemolysate was added sufficient to produce a fractional cell water volume, from about 0.000095 to 0.001. Carbon backbone structure of the omega loop such that acrylodan side chain at positions 81 and 84 become exposed to hydrophilic environment. The fact that substitutions at both positions yield acrylodan spectra with equivalent emission maxima after ligand binding suggest a conformational involvement of the entire loop. Similar to fasciculin, small ligands that bind to active center produce a similar strain. All of the small ligands, whether reversibly bound or covalently attached, elicit marked changes in acrylodan emission with the largest spectral shift seen for E84C, an intermediate value seen for E81C and only small change observed for L76C. In each case the conformational change induced by the ligand causes the acrylodan to move into a region of higher dielectric constant, presumably being more solvent exposed. The pattern is remarkably consistent among the ligands, and only the small organophosphate when conjugated induces a shift of smaller magnitude. A likely explanation for the observed conformational changes is that ligand binding to the active center induces gorge closure, which is mediated throughout the omega loop. The strain placed on the -carbon backbone upon gorge closure causes the side chains to shift positions and become exposed to hydrophilic environment.
Hold the leg relative to flexion-extension and internal-external rotation. This provides the ability to position the small incisional "window" exactly where it is needed relative to the anatomy to be resected. The non-operative limb is supported appropriately in a padded leg holder. The TGSTM robotic arm is registered in the sterile operative field and then positioned appropriately for access to the patient's knee. The patient's leg is then prepped and draped. Bicortical femoral and tibial pins are inserted and the optical tracking arrays applied. Operative field visualization is confirmed. Prior to knee dissection, knee alignment, mechanical axis and range of motion are captured using the TGSTM system software. The kinematic relationship of the implant to the pre-operative plan can then be evaluated through a full range of motion. Adjustments to the plan can be made intra-operatively as deemed appropriate. A anterior medial incision is made from the inferior medial aspect of the patella to the proximal aspect of the medial tibia. The medial capsule is then incised and the anterior horn of the medial meniscus is resected. A soft tissue retractor is used to evaluate the other knee compartments and the integrity of the ACL. The suprapatellar synovial band is released to allow patellar mobilization. The knee axis and femoraltibial registration points are captured and then the TGSTM integrates the pre-operative CT scan and registration points. Once completed, the TGSTM is poised to provide the surgeon an intelligent interactive instrument. The tibial footprint is prepared and the TGSTM optical probes are then used to confirm and verify precision of the prepared bone and cartilage boundaries. The trial implant is then inserted and the optical probes are and cefprozil.
