Used here ; or by extracellular enzymes released into the , medium. Since rat HDL, which contains apoE, may bind to both the LDL receptor and to the HDL receptor, we tested whether specific binding of lZ5I-labeledrat HDL was affected by cholesterol feeding or cholestyramine treatment. Fig. 6 demonstrates that the binding of lZ5I-labeled rat HDL was similar in these groups, which suggests that neither LDL nor HDL3 receptor activity in the mucosal cells was affected by these treatments. Since cholestyramine and surfomer treatment produces a long term inhibitory effect on cholesterol absorption which consequently influences sterol synthesis, we next compared the acute effect on cholesterol absorption caused by biliary diversion. We isolated mucosal cells from rats that had undergone biliary diversion for about 16-20 hr which eliminates more than 80% of the total bile acid pool 23 ; . There was, however, no change in the capacity of cells to bind 1251-labeled HDL3 Fig. 7 ; , even though this manipulation had caused a dramatic increase in the rate of intestinal sterol synthesis. As the intestinal epithelium is composed of upper villus cells where lipid absorption takes place 24 ; and lower villus to crypt cells where sterol synthesis mostly occurs 14 ; , we tested lz5I-1abeledHDL, interaction to these two groups of cells Fig. 8 ; . However, specific binding.

That the impairment of 7a-hydroxylation of cholesterol is correlated with, and presumably due to, the reduced liver cell mass. Since 7a-hydroxycholestero1is precursor to both cholic and chenodeoxycholic acid, the reduction of its synthesis, were this the sole defect, should affect both primary bile acids equally. Since this is not the case, it seems that possibly multiple defects, including the 7a-hydroxylation step, at different levels of the pathway could be responsible of the preferential defect of cholic acid synthesis 22-24 ; . Work on laboratory animals such as the rat has led to the presently accepted view that the 7a-hydroxylation of cholesterol is the rate-limiting step in the pathway from cholesterol to bile acids and that it is feed-back-inhibited by bile acids returning to the liver 4, 25 ; . This undoubtedly may hold true also for humans 26 ; , but information on the relation between overall bile acid synthesisand 7ahydroxylation of cholesterol is very limited in man. In this study the administration of cholestyramine to one subject produced a threefold increase of the hydroxylation rate. A possible source of error should be considered in conditions of stimulated sterol synthesis. As observed by Redinger, Chow, and Grace 27 ; in bile fistula primates, bile acid synthesis evaluated with 14C02breath analysis was greatly underestimated. This is probably due to the preferential utilization, especially in conditions of stimulated bile acid synthesis, of newly synthesized microsomal cholesterol with which the tracer could mix incompletely. Even when our results are in close agreement with data obtained with the isotope dilution method 19 ; , we cannot exclude that our technique could have underestimated, to some extent, the rate of 7a-hydroxylation under these conditions. It is apparent anyway that the interruption of the enterohepatic circulation does derepress the synthesis of bile acids. However, the mechanism controlling the responsible enzyme, the microsomal cho1284.
Therefore, it would be expected that a dosing schedule of cerivastatin sodium given at bedtime and cholestyramine given before the evening meal would not result in a significant decrease in the clinical effect of cerivastatin sodium and chooz.
Manner Situations in which way or manner plays a role. This may be Manner incorporated in a dynamic situation, e.g. ways of movement such as walk, swim, fly, or the static Property itself: e.g. manner, sloppy, strongly, way. Manner as a SituationComponent applies to many specific BCs that denote a specific way or manner in which a Dynamic event takes place. Typical examples are ways of movement. General BCs that only refer to Manner as such and not to some specific Situation are Static nouns such as manner, way, style. Location Situations involving spatial relations; static e.g. level, distance, separation, course, track, way, path; something changes location, irrespective of the causation of the change; e.g. move, put, fall, drop, drag, glide, fill, pour, empty, take out, enter. Location is typically incorporated in Dynamic BCs denoting movements. When combined with Static it clusters nouns that refer to Location Relations, such as distance, level, path, space. A Location Relation holds between several entities and cannot be seen as a property of single entity. This makes it different from Place, which applies to a 1stOrderEntity that functions as the location for an event or some other 1stOrderEntity. Experience Situations that involve an experiencer: either mental or perceptual through the senses. Situations with the TC Experience involve the mental or perceptual processing of some stimulus. In this respect there must be an experiencer implied, although it is not necessarily expressed as one of the arguments of a verb it could be incorporated in the meaning ; . Typical Experience BCs are: to experience, to sense, to feel, pain, to notice. Experiences can be differentiated by combining it with Physical or Mental. Physical Experiences are external stimuli processed by the senses: to see, to hear. Mental Experiences are internal only existing in our minds: desire, pleasance, humor, faith, motivation. There are many examples of BCs that cannot be differentiated between these, e.g. pain that can be both Physical and Mental. Another interesting aspect of Experiences is that there is unclarity about the dynamicity. It is not clear whether a feeling or emotion is static or dynamic. In this respect Experience BCs are often classified as SituationType, which is undifferentiated for dynamicity. Existence Situations involving the existence of objects and substances; Static states of existence e.g. exist, be, be alive, life, live, death; Dynamic changes in existence; e.g. kill, produce, make, create, destroy, die, birth.

Working behind the scenes, she made sure daily life at the Duke Eye Center ran like clockwork. As manager of clinic services at the main Eye Center on campus and three Triangle satellites, her job impacted many--from patient to doctor to clinical staff. She has seen the Eye Center faculty grow from five doctors to more than 50. Carolyn Vaughan retired from the Eye Center on May 31, after an ophthalmology career that began in 1973. A co-worker announced that she was leaving to become nurse manager at the then-new Eye Center and asked Vaughan if she was interested in changing careers from respiratory therapy to ophthalmology. "I loved it from minute one! I could not study or learn enough, " Vaughan says. The transition was natural. "I had always been in some field of medicine, taking care of patients as an LPN and as a respiratory therapy technician, " she says. Vaughan became one of the first certified ophthalmic technicians COT ; at Duke in 1979. For nine years, Vaughan worked for the Eye Center's neuro-ophthalmologists. Then, she and ophthalmologist Charles Sydnor, MD, left Duke to join Paul Abernathy, MD, a former Eye Center resident, to establish the Alamance Eye Center. Vaughan says it was Sydnor's belief in her abilities as a leader that gave her the confidence to do what needed to be done. "I trained 14 assistants to the COT level, assisted in the planning and building of the Alamance Eye Center, and managed the medical side of the practice for the next 10 years, " she says. Four years later, following a stay in New Mexico, Vaughan interviewed for a part-time COT position in the Comprehensive Service at the Eye Center. Ultimately, that post led to Vaughan's becoming manager of clinical services. She quickly found new mentors at the Eye Center. Vaughan has fond memories of the physicians and administrators she worked with over the years and cinacalcet.
Background & Aims: Treatment of intrahepatic cholestasis of pregnancy with ursodeoxycholic acid appears promising, but data are limited so far. The aim of this randomized study was to evaluate the efficacy and safety of ursodeoxycholic acid in comparison with cholestyramine. Methods: Eighty-four symptomatic patients with intrahepatic cholestasis of pregnancy were randomized to receive either ursodeoxycholic acid, 8 10 mg kg body weight daily n 42 ; , or cholestyramine, 8 g daily n 42 ; , for 14 days. The primary end point was a reduction of pruritus by more than 50% after 14 days of treatment as evaluated by a pruritus score. Secondary end points were outcome of pregnancy, reduction of serum aminotransferase activities and serum bile acid levels, and drug safety. Intention-to-treat analysis was applied. Results: Pruritus was more effectively reduced by ursodeoxycholic acid than cholestyramine 66.6% vs 19.0%, respectively; P .005 ; . Babies were delivered significantly closer to term by patients treated with ursodeoxycholic acid than those treated with cholestyramine 38.7 1.7 vs 37.4 1.5 weeks, respectively, P .05 ; . Serum alanine and aspartate aminotransferase activities were markedly reduced by 78.5% and 73.8%, respectively, after ursodeoxycholic acid, but by only 21.4%, each, after cholestyramine therapy P .01 vs ursodeoxycholic acid ; . Endogenous serum bile acid levels decreased by 59.5% and 19.0%, respectively P .02 ; . Ursodeoxycholic acid, but not cholestyramine was free of adverse effects. Conclusions: Ursodeoxycholic acid is safe and more effective than cholestyramine in intrahepatic cholestasis of pregnancy.

Cated that Ti was not strongly affected by the lipid composition of the membrane. S, may be increased by the presence of PE. Both steady-state anisotropy measurements and timeresolved anisotropy measurements of cholestatrienol were used to probe sterol behavior in three biological membranes: bovine rod outer segment ROS ; disk membranes, human erythrocyte plasma membranes, and light rabbit muscle sarcoplasmic reticulum membranes. In the ROS disk membranes the value for S, was marginally higher than in the PC membranes, perhaps reflecting the influence of PE. The dramatic difference noted was in the value for Ti. In both the ROS disk membranes and the erythrocyte membranes, was one-third to one-fifth of in the phospholipid bilayers. This Ti result may reveal an influence of membrane proteins on sterol behavior and cisplatin.
