It seems to me that we ought to provide at least one experience that allows the student to travel the health care road alongside the patient. This might involve sitting at a patient's bedside for 24 to 48 hours, as would a close member of the family, or working with a patient from home to clinic to pharmacy to home again. These kinds of experiences would allow the student to observe the ebb and flow of care; comprehend the jagged nature of a patient's health status; get a sense of the worry, fear, sadness, and impatience the patient and their families experience as they navigate the health care system when they are most vulnerable; identify system solutions that might enhance care; and develop a personal approach to care that maximizes continuity of care and a human touch. As clinician educators, we can model direct patient practice for our students and provide them with the confidence to work at the bedside as well as from medical records. We can make students more aware of how much they can learn from and through the patient. As we guide our students through their practice experiences, we can bring into bold relief the lessons they can glean from conversations with patients and their families and through their own clinical observations. In doing so, we will imbue them with the essence of individualized pharmacotherapy. We can encourage students to modify our delivery systems and their approach to care with the patient's perspective in mind. For every less-than-ideal situation that our students encounter, we can challenge them to think actively about how things could be done differently if they were in charge or how they might modify their own practices in ways that enhance patient experiences. Finally, let's ask our students to think about and use the lessons they learn at the bedside to teach the families and friends of patients how to effectively advocate for best care. Lessons for Loved Ones Friends and Family Here are some of the lessons I have learned as Carole's sister. Become a member of the health care team. Although I have exhorted us all to get to know the patient beyond what we find in the medical record, I 5 acknowledge that even the best systems and most competent clinicians will sometimes fail in providing optimal care for our family members. Certainly, we cannot expect clinicians to know our loved ones as intimately as we do. Therefore, we must ourselves become members of the health care team and serve as advocates for our family members when they cannot. Carole and our family have learned that there is no predictable or natural progression in her health status, only better or worse. Here's how Carole views her situation.

There is a path you can follow, a very definite sales trend line whose course resembles that of an accelerating missile, beginning at a point some 40 years ago, climbing steadily and surely, almost without variance, to the present day. If you followed that path of SUV sales backwards, you would find yourself at the very dawn of the modern SUV market, where it all began, in the fall of 1962 . and you would find yourself face to face with the amazing Jeep Wagoneer. Few vehicles can maintain their edge after many years in production. Sure, cars and trucks usually become a little better the second or third year they're built, as assembly workers become better acquainted with their jobs, and engineers find little ways to improve things. But over a longer period, new technology appears, new processes are developed, and older vehicles tend to get left behind. So how to explain the Jeep Wagoneer? On its debut it was recognized as a breakthrough: a bold, revolutionary design. Three years later, the motor press was still singing its praises -- Track & Traffic said it reminded them of "an All-American football player -- tough but clean-cut. From its fashionable and attention-grabbing exterior to the luxurious finish inside." Six years after that, the Wagoneer continued to hold its place as everyone's favorite four-wheel-drive vehicle, Four Wheeler calling it the "limousine of fourwheelers." Amazingly, after 10 years in production, Pickup, Van & 4WD magazine declared, ". Wagoneer is the best four-wheel drive in the world." Surprised? Then consider this -- 16 years after introduction, Four Wheeler called Wagoneer the Rolls Royce of the four-wheel-drive world." "The results presented at AACR provide new insights into the mechanism of action of Targretin in NSCLC by uncovering synergistic activity of Targretin together with different commonly used chemotherapeutic agents and by characterizing a unique action of Targretin to prevent or reverse acquired drug resistance to paclitaxel and other chemo agents, " said Andres NegroVilar, M.D., Ph.D., Ligand's executive vice president for research and development and chief scientific officer. "We believe these insights on mechanism are particularly relevant to our combination studies SPIRIT I and II frontline ; and our ongoing and planned studies in second and third line." Targretin Prevents or Reverses Acquired Taxol Resistance in NSCLC Cancer Cells In Vivo and In Vitro ; Paclitaxel Taxol ; is an important anticancer agent for the treatment of NSCLC, breast and many other types of cancer. Its extended utility, however, is limited by the development of acquired drug resistance by tumor cells, a phenomenon that also affects other taxanes and chemotherapeutic agents. The potential role of Targretin in preventing or reversing taxol resistance is addressed in a study abstract #2144 ; being presented Monday, March 29, 2004. Ligand's scientists developed a model of taxol resistance NSCLC cell lines and demonstrated that using different regimens of sequential and or continuous combined treatment of paclitaxel with Targretin resulted in the prevention and or reversal of acquired resistance to paclitaxel. Resistant NSCLC tumor cells not only did not respond to additional paclitaxel treatment but also showed that the acquired resistance also affected other commonly used chemo agents such as vincristine and doxorubicin. All these agents have been shown to be substrates for the P-glycoprotein pump. Development of resistance was confirmed by the increased expression of the multi drug resistance gene MDR-1 and by the increased efflux activity of PgP. The results further showed that Targretin prevented resistance and cross-resistance by suppressing the expression of the MDR-1 gene and the activity of the P-glycoprotein pump in NSCLC cells. In vivo studies showed that Targretin and paclitaxel combined treatment provided a much better antiproliferative response in NSCLC tumors and that the combination of paclitaxel plus Targretin but not paclitaxel alone significantly decreased tumor growth in paclitaxel-resistant tumors. Synergistic Activity of Targretin and Cytotoxic Agents in Arresting NSCLC Growth An additional study being presented at AACR abstract #1989 ; demonstrates that a combination of Targretin and different therapeutic agents i.e., paclitaxel, taxotere, etc. ; produced a combined arrest of NSCLC cell tumor growth larger than each single agent alone. The combined activity of Targretin and paclitaxel was synergistic and the observed effects were seen across a panel of 10 different NSCLC cell lines representative of different tumor histologies seen in NSCLC patients. "These data support our clinical observations in Phase I II trials that Targretin has beneficial effects when used in combination or sequentially with cytotoxic agents in advanced Stage IIIb and IV ; NSCLC, " said Negro-Vilar. "Ligand has recently completed two Phase III trials, Spirit I and Spirit II 1235 patients ; , combining Targretin with two commonly used chemo regimens, cisplatin + vinorelbine or carboplatin + paclitaxel, respectively. Patients are currently being followed for survival until the pre-determined trigger point for statistical analysis. Targretin Prevents and Overcomes Acquired Drug Resistance in Advanced Breast Carcinoma In a parallel study abstract #2146 ; being presented on Monday, March 29, 2004, Ligand scientists demonstrated the activity of Targretin in preventing or reversing acquired drug resistance as well as intrinsic drug resistance in a human advanced breast carcinoma cell line, MDA-MB-231. Cells resistant to paclitaxel or doxorubicin showed a higher degree of invasiveness and angiogenic activity than those whose resistance had been prevented or reversed by concomitant treatment with Targretin. These results correlated with in vivo studies in which Targretin added to chemotherapy resulted in superior reduction of tumor growth to chemotherapy alone. Overall, the data clearly demonstrates the potential for Targretin to prevent or reverse acquired or intrinsic drug resistance in advanced breast carcinoma by interfering with tumor cell progression to a more malignant drugresistant, invasive phenotype and clofarabine.

Ble-blind evaluation is virtually impossible with most foods but is possible with lactose. An uncontrolled study found that 59 percent of 44 lactose-intolerant men experienced symptoms after drinking 240 ml of milk.10 In contrast, in a series of double-blind studies, Scrimshaw's group found that most people with lactose malabsorption not selected for the self-reported severity of their intolerance ; tolerated 240 ml of a chocolate drink containing 12 g of lactose.12-14 Our study extends these observations to people who believed themselves to be extremely intolerant to very small doses of lactose, such as the amounts contained in milk used with coffee or cereal. In addition, we attempted to evaluate the usual pattern of milk ingestion by assessing the severity of a variety of symptoms over a one-week period during which 240 ml of milk was ingested daily. In previous studies that evaluated the responses to single doses of lactose in a chocolate drink, 12-14 the chocolate may have had an independent effect on symptoms.25 We analyzed each symptom independently, unlike investigators who only reported symptoms as present or absent26 or as the sum of symptom intensity.27 Our finding that 240 ml of milk was not associated with a significant increase in the severity of bloating, abdominal pain, or flatus suggests that people frequently misattribute a variety of abdominal symptoms to lactose intolerance. A larger study might have uncovered some differences. Nevertheless, the symptoms that might be caused by lactose are unlikely to be substantial, particularly in view of the expectation of many subjects before the study that the distress caused by ordinary milk would preclude their completing the study. In most people with lactose malabsorption, the ingestion of 50 g lactose in a single dose produces symptoms, but that is equivalent to drinking a liter of milk. Future studies should address tolerance of 240 ml of milk consumed throughout the day with meals. A variety of lactose-digestive aids are available over the counter. Overnight incubation of milk with 5 or 15 drops of a liquid lactase preparation per 240 ml of milk is recommended to produce 70 percent or 100 percent hydrolysis of lactose, respectively. The cost of this treatment, based on the price of a bottle of the preparation in Minnesota, is about 6 cents per 240 ml of milk 70 percent hydrolysis ; or 18 cents per 240 ml 100 percent hydrolysis ; . Lactase is also available as a tablet that can be taken with a lactose-containing food. The cost of the recommended dose two tablets ; ranges from 30 cents to 60 cents in Minnesota, depending on whether a carton of 100 or 12 tablets is purchased. In many areas, commercially prehydrolyzed milk is available at a cost of about 36 cents per 240-ml serving. The price of milk in Minnesota is about 19 cents per 240 ml. In our area, the use of lactose-digestive aids increases the cost of milk by a minimum of about 6 cents per 240-ml serving 70 percent hydrolysis of lactose by lactase drops ; to a maximum of about 41 cents per 240-ml serving lactase tablets purchased in small quantities ; .
