Relatively inexpensive hematofluorometer 3, 23, 25 ; , but because of the finite lifetime of affected erythrocytes and the resulting short half-life of lead in blood, these laboratory-based measurements can assess only ongoing or recent lead exposure, within a period of a few weeks to months. If longer-term exposure is suspected, a somewhat more accurate picture of the overall lead body burden can be provided by the provocative chelation test 3, 26, 27 ; . After administration of a chelating agent such as calcium disodium ethylenediaminetetraacetic acid CaNa2EDTA ; , lead is mobilized from the soft tissues and excreted in the urine, where it is quantified by atomic absorption 2, 3 ; . Increased urine lead concentrations signify prolonged lead exposure and indicate the need for additional treatment. Determination of lead in hair has also been considered for assessment of chronic exposure, but variability attributable to differing hair types and textures, as well as the high possibility of external contamination from dustborne lead, can render such measurements difficult to interpret 7, 17 ; . True assessment of cumulative lead body burden should use measurements of bone lead, where most of the body lead burden is concentrated. Historically, such measurements involve wet chemical digestion of bone material followed by atomic absorption-based measurements and were limited to specimens from cadavers or bone biopsy samples. Measurement of tooth lead is possible by similar means. Like bone lead, tooth lead increases as a function of age and exposure 7, 28, 29 ; . Such measurements have been widely performed on shed primary teeth to assess lead exposure in children 6, 30 ; . Measurements of lead in the permanent teeth of adults are less practical because of the highly invasive nature of sample collection, and are rarely performed.

Recommendations for Management of AF in Patients With HCM Class I Treat patients with HCM who develop AF with oral anticoagulation INR 2 to 3 ; recommended for other high-risk patients for prevention of thromboembolism. Level of Evidence: B ; Class IIa Antiarrhythmic medications to prevent recurrences. Available data are insufficient to recommend one agent over another in this situation, but disopyramide and amiodarone are generally preferred. Level of Evidence: C ; 7. Pulmonary Diseases. Supraventricular arrhythmias, including AF, are common in patients with chronic obstructive lung disease 575, 576 ; and have adverse prognostic implications in patients with acute exacerbations of chronic obstructive pulmonary disease 577 ; . Treatment of the underlying lung disease and correction of hypoxia and acid-base imbalance are of primary importance. Theophylline and beta-adrenergic agonists, which are commonly used to relieve bronchospasm in these patients, can precipitate AF and make it difficult to control the rate of ventricular response. Beta-blockers, sotalol, propafenone, and adenosine are contraindicated in patients with bronchospasm and wheezing. Rate control can usually be achieved safely with calcium channel antagonists 578 digoxin offers no advantage over calcium channel antagonists in this situation. Pharmacological antiarrhythmic therapy and electrical cardioversion may be ineffective against AF until respiratory decompensation has been corrected. Intravenous flecainide may be efficacious in restoring sinus rhythm in some patients 232 ; . Electrical cardioversion may be attempted in hemodynamically unstable patients. In resistant cases, AV nodal ablation and ventricular pacing may be necessary to control the ventricular rate. The role of anticoagulation in patients with AF due to chronic obstructive lung disease has not been studied specifically. Recommendations for Management of AF in Patients With Pulmonary Diseases Class I 1. In patients who develop AF during an acute pulmonary illness or exacerbation of chronic pulmonary disease, correction of hypoxemia and acidosis are the primary therapeutic measures. Level of Evidence: C ; 2. In patients with obstructive pulmonary disease who develop AF, a calcium channel antagonist agent diltiazem or verapamil ; is preferred for ventricular rate control. Vividly remember a day back in 2002 when I told one of the other "moms" in the school yard that I had purchased my first paper trimmer and some patterned paper. I remember the passion in my voice as I described, in detail, all of the products that I had seen at the store. Before we knew it, play dates were planned where our kids could play, and so could we! A few short months later found us meeting on a regular basis on Monday evenings in my home. I supplied the space and we took turns with the dessert! Not only have we all maintained a great friendship; we have continued to share our passion with each other. Most Kurdish parties currently advocate a careful and modest approach, focusing on cultural and educational issues and avoiding antagonizing the government. A smaller number are beginning to favour a more radical strategy, arguing that decades of caution have brought little improvement. Among the many divisions between Syrian Kurds is also a complex sense of identity. Most Kurds are proud of their Kurdishness and a considerable number have fought in Kurdish risings in Iraq and Turkey.
