A 30-year-old white woman has a 72-hour history of a pruritic rash on her arms and chest. This is the third episode of the rash this summer, and each episode began in the evening after she had spent less than 30 minutes by a swimming pool in the morning. She has no history of sensitivity to sunlight and is taking no medications. Both the rash and the pruritus are improving, but she still has discomfort. How should this woman be evaluated and treated?
Median, 53.5 years. Median, 6 episodes in 12 months. Median, 0.2 episodes in 12 months. Patient died in November 1995. Patient died in December 1992. Episodes were mild, with less than 20% increase in hematocrit value from baseline and hospitalization that lasted less than 48 hours and required neither intensive care unit management nor intubation or central venous monitoring. * Two episodes were mild and one was of usual severity.

Another Battle" Series is influenced by the snapshots of patriotism and heroism extolled in the old movies talking about safeguarding the motherland that I watched in childhood. The heroes gave their lives for the liberation of new China in movies like "Landmine War", "Guerrilla War", "Three Liberation Wars", etc. I used to dream about becoming a soldier, too. Up until now, this dream has not been realized. Nowadays economic reconstruction is like a war that progresses so rapidly and intensely. In this war, we have to face contradictions from both Chinese ancient civilization and modern western civilization. I call these contradictions Another Battle. In this battle of "defending our country", without fire and gunshots, I portray myself as a defeated commander. It looks there is no winner at all in this battle. My new work "Competition" focuses on the power of ads and the misconceptions that ads can create. For this photo work, I constructed a chaotic backdrop where over 20 people are depicted in a frenzy of competition with some even fist fighting while jostling for ad positioning on a huge billboard advertisement; this struggle for the most optimal outdoor ad placement is perceived as inevitably bringing power and influence. The struggle for ad placement in public space in China is not unlike a battlefield strewn with casualties after a pitched battle for power. Today one brand wins. The next day, its competitor will replace it with better positioning on public spaces. Every day, new ads go up, and old ones fall down, scattered in pieces, and discarded on the ground under newly erected billboard advertisements. In this work, I've constructed a huge wall, that stands about 14 meters high and 40 meters across, and I fixed over 600 pieces of paper 110 x 90 cm each ; on which wrote in traditional Chinese ink brush style in some instances and in felt tip pen and magic marker in others, a random selection of slogans and phrases from the advertisements that bombard us here every day. These ads include both domestic and international information about companies and famous brands, such as the lease of houses, education programs, restaurants, foot massage, etc. Everything is advertised, from items as big as airplanes BOEING ; or as small as vinegar and condoms. On my gigantic wall, I make the fight for advertising as fierce as a struggle for military power, with inevitable casualties on the battlefield. I've used around 3000 varieties of products and services on my wall to show off the allure of this mass advertising campaign that surrounds us. | Indinavir side effectsAfter a brief hiatus and soul searching, I'm pleased to present this issue of WISE Words, with content changes and a brand new look! WISE Words is the publication of Project WISE, an interdepartmental program of Project Inform focused on HIV AIDS treatment information and advocacy for women. I honored to be its new coordinator! One new feature of WISE Words will be an ongoing public policy forum on issues affecting HIV-positive women's lives. It provides a means of action for women to get involved in current policy debates. Another new feature is opening up A Word from WISE to our readers who wish to share informative treatment related experiences. Finally, each issue will include a glossary of key concepts and terms noted in bold italics ; . In this issue we feature an overview of some gynecological GYN ; complications associated with HIV disease. It's important to consider that GYN health may be telling us something about the status of our immune system. When conditions become more difficult to treat, it probably means the immune system is weakening. The need for regular exams and prompt treatment of problems cannot be overstated. For women currently taking or considering an indinavir Crixivan ; twice daily regimen, new study results show that an experimental twice daily dosing regimen is not as effective in keeping viral load "undetectable" as the three times daily regimen. People using a twice daily regimen are encouraged to switch to a three times daily regimen or at least to discuss the issue thoroughly with their doctors. Finally, information on viral load results and women are starting to come to light. Two studies suggest that women progress to symptoms of HIV disease with lower viral load levels than men. It's unclear what this preliminary information means for women, but we are contacting researchers for further exploration of this issue. The next issue of WISE Words, arriving in early 1999, will review the information to date. For now, remember that CD4 + cell counts, not viral load tests, are the primary means of predicting the immediate risk of acquiring opportunistic infections. These compounds are highly stable at room temperature, but the magnetic and spectral properties that they show depend on the anion they bind with [8]. For example, the Ni TAAB ; X2 compounds show tetragonal geometry. The study of the incorporation of substituent groups at the macrocyclic ligand is quite interesting because of the possibility of using such systems in nanoelectronics systems. These compounds are of particular interest in the study of low-dimensional metals and semiconductors as one may bridge macrocycle nuclei with linear bidentate axial ligands, which suggests a model for low-dimensional polymeric conduction in macrocycles [8]. Because protease inhibitors PIs ; produce favorable long-term suppression of viremia, elevation of CD4 lymphocyte count and retard disease progression with improved survival when combined with other antiretroviral agents, they form an integral part of Triple therapy in HIV infection. Interaction of PIs is mainly due to inhibition of Cytochrome P450 enzymes CYP3A4 and CYP2D6. Potency of enzyme inhibition varies among different PIs, ritonavir being the most potent one. PIs other than ritonavir mainly inhibit CYP3A4 while ritonavir inhibits CYP2D6 enzyme. PIs have been reported to increase the concentration of bupropion, nefazodone and fluoxetine to toxic levels and to increase the concentration of desipramine by 100 to 150 percent[15]. However, one pharmacokinetic study concluded that dose of ritonavir need not be reduced in patients concomitantly receiving fluoxetine[16]. In patients receiving antiretroviral therapy along with fluoxetine, serotonergic syndrome has been reported and this should be suspected in patient on any serotonergic drug taking cytochrome P450 enzyme inhibitors such as PIs, non-nucleoside reverse transcriptase inhibitors and grapefruit juice. Serotonergic syndrome is characterized by mental status change, autonomic dysfunction and neuromuscular abnormalities[15]. PIs may also increase the plasma level of some benzodiazepines such as alprazolam, midazolam, triazolam and non-benzodiazepine hypnotic zolpidem[16]. Ritonavir, nelfinavir and amprenavir are also moderate inducers of some of the hepatic enzymes including CYP3A4. This may complicate the interaction on long term and therapeutic drug monitoring should be taken as guide for antidepressant therapy and dosing when required. Furthermore, since generalized anxiety is very common in depressives having HIV infections, along with antidepressants anti-anxiety agents may be required. Significant interactions between antidepressants and anxiolytics include-: tendency of fluvoxamine to increase the plasma concentration of oxidatively metabolized benzodiazepines, and sertraline along with fluoxetine increasing the level of benzodiazepines. Nefazodone potentiates benzodiazepines other than lorazepam and oxazepam[16]. It has also been shown to increase plasma level of indinavir and efavirenz[8]. These interactions may be important in HIV patients because the higher level of benzodiazepine may mask the improvement obtained by the use of antidepressants in terms of somatic as well as psychological symptoms. Fluvoxamine and fluoxetine increase the plasma level of most of PIs along with that of delaverdine and efavirenz. On the other hand, nevirapine decreases the plasma level of both the antidepressants [8] and iressa.
