Nasal route is an alternative and convenient route for administration of high potency drugs with low oral bioavailability because of their extensive gastrointestinal breakdown and high hepatic first-pass effect. Drugs of a lipophilic nature are difficult to deliver through the nasal route due to their poor water solubility. Large hydrophilic drugs like peptides and proteins show insufficient nasal absorption. One more drawback of this route is large interspecies differences exist in the nasal absorption of drugs. Side effects such as nasal dryness and blood in mucus were attributed to the viscosity of the formulation vehicle and high frequency of administration. Dimethyl-b-CD and 2-hydroxypropyl-g-CD has shown an enhanced and sustained level of morphine in the plasma and cerebrospinal fluid 45 ; . The current transdermal therapeutics market is segmented into traditional formulations like gels, advanced delivery systems, such as patches, and novel physical technologies like microporation, iontophoresis, and sonophoresis. Key management objectives are to provide lasting symptom relief and reduce the risk of disease progression to acute urinary retention or BPH related surgery. Over the past decade the prevalence of prostatic surgery has decreased whilst medical therapy has become increasingly popular. There are two lines of medical attack on the problems posed by benign enlargement. The class of drugs known as 5 alpha-reductase inhibitors work to shrink the prostate by inhibiting the enzyme that causes growth of prostate cells, whilst alpha blockers work by relaxing the smooth muscle tissue at the bladder neck, thus reducing outflow obstruction. The guidelines recommend the use of 5 alpha-reductase inhibitors in men who are judged to be at risk of progression whilst alpha blockers are recommended for men at lower risk or in combination with a 5 alphareductase inhibitor if the symptoms are particularly bothersome and levetiracetam.
