Phencyclidine 1 phenylcyclohexylpiperidine, PCP ; was originally used as horse tranquilizer is now a popular drug of abuse. This drug is structurally related to ketamine and was thought to have human use as an intravenous anesthetic. This drug can be administered by smoking, nasal insufflation, oral intake or by intravenous injection. Its primary effects are lethargy, hallucinations, loss of coordination and disorientation. Currently, a number of designer street ; drugs have been synthesized by similar chemical reactions. They include Phenylcyclohexylpyrrolidine PHP ; , Phenylcyclopentylpiperidine PCPP ; and Thienylcylohexylpiperidine TCP ; see structures below ; . Blood levels for PCP vary from 0.007 to 0.24mg L 1 ; . The plasma half-life has been estimated at 1113 hours 2 ; and 7-46 hours 3 ; . In cases of severe poisoning a half-life of 1-4 days may occur 4 ; . The metabolism of PCP indicates 2 identified inactive metabolites-4-phenyl-4 piperdinocylohexanol and 1 1-phenylcyclohexyl ; -4-hydroxypiperdine which are further metabolized to glucuronide conjugates that are found in the urine. Intravenous dosing indicated 30-50% of the dose was found in the urine as unchanged PCP within 72 hrs 5 ; . One problem in the ingestion of the street variety of PCP is the starting precursor found in 20% of samples can release cyanide "In Vivo". This can have serious toxicity issues 6-8.

The data pre sented in table 7 show that the vitamin activity of MCA isomers I and III was very high and comparable to the activity of the mixed MCA for the developing embryo. Isomer II, however, exhibited no vitamin activity and may have been weakly inhibi tory because it reduced hatch from the level of the saline control. This reduction, however, was not statistically significant. In contrast to the results with the chick embryo, MCA isomer I was inhibitory for the E. coli mutant and MCA isomer III had neither an inhibitory effect nor vitamin activity. Isomer II proved to be the most potent inhibitor in the bacterial system. The relative potencies of the MCA isomers determined in this study with the E. coli mutant agree with the data of Keleman et al. 8 ; although the absolute values for the inhibition indices are smaller in our study of the individual isomers. The data support the conclusion that two of the three MCA isomers have vitamin activity for the chick embryo but that a third does not have ac.

Velcade is an approved treatment option for patients who have received at least one prior therapy and various Velcade regimens are being utilized in relapsed or refractory disease. Velcade was evaluated in a randomized, multicenter Phase III trial known as the APEX trial. This trial compared Velcade with high-dose dexamethasone in 669 patients with relapsed and refractory myeloma who had received one to three prior lines of therapy. APEX was ended early due to significant improvement in disease progression in patients receiving Velcade. At that time, patients receiving high-dose dexamethasone had the option to immediately crossover to Velcade treatment. Initial results of the APEX trial show a 78% improvement in the median time to disease progression TTP, approximately 3 months ; with Velcade over dexamethasone Figure 5 ; . Treatment with Velcade was associated with statistically significant improvement in 1year survival 80% vs. 60% ; and median overall survival 30 mos. vs. 24 mos. ; over dexamethasone. This is the first and largest randomized study demonstrating a survival advantage of a single agent in relapsed myeloma. Response rates with Velcade were improved over those seen with dexamethasone both at the time of early termination of the study 8.3 months ; and at an updated analysis 22 months ; Figure 6 ; . The overall incidence of serious and Grade 4 side effects in the APEX trial was similar in both arms. The incidence of Grade 3 events mostly hematologic effects ; was higher in the Velcade arm. Patients receiving dexamethasone experienced increased risk of psychiatric problems, infections, and bone disease while patients receiving Velcade experienced increased gastrointestinal effects, nerve damage, low blood counts, and transient reversible decrease in platelet count and vesicare.
